This
program is the first step in MEOC’s care coordination service and handles all
referrals for assistance made to MEOC.
MEOC receives referrals for assistance
from many sources including older persons themselves, family members, neighbors,
ministers, hospitals, home health agencies and others. This program determines
the needs of individual older persons and then links the person in need with the
appropriate service(s). A well-trained case manager visits the person, gathers
information and assists in linking the person and the family with the services
needed. MEOC staff are familiar with all community services, not just MEOC
services, and link individuals to all services needed and available within the
community.
