This
service is for
Medicaid-eligible persons who have been screened and have met the requirements
for nursing home placement but who, with assistance, can remain in their own
homes. R.N.s supervise certified
geriatric aides who assist patients with bathing, dressing, feeding, walking,
medication monitoring, meals, laundry, shopping, and housekeeping.
This service is also available to non-Medicaid persons as a private pay service.
Fees:
Current Medicaid reimbursement rate.
Eligibility: Persons in need of assistance with 3 or more activities of
daily living.
Intake
Procedure: Walk-ins and telephone referrals accepted.
Service
Area: Lee, Scott and Wise Counties, and The City of Norton.
