The health and social status of each participant is assessed by a team of professionals, including a physician, nurse, social worker, physical or occupational therapist, pharmacist, dietician, home care coordinator, and recreation therapist.  The assessment includes medical, dental, dietary, and behavioral needs, medication use, language, physical and cognitive abilities, socialization and family support, home environment, and participant and caregiver preferences for treatment.
