Program of All-Inclusive Care
for the Elderly

 PACE PROVIDES LOVING CARE TO PARTICIPANTS
AND PEACE OF MIND TO THEIR FAMILIES


Most PACE services are provided by the Interdisciplinary Team.  The Interdisciplinary Team includes:

  • primary care physician
  • registered nurse
  • dietician
  • social worker
  • bus driver
  • home care coordinator
  • personal care aide
  • physical therapist
  • occupational therapist
  • recreation therapist

The Team may also include a pharmacist and speech therapist.  The Team writes a plan of care for each participant, provides services according to the plan, and approves any changes  in the plan.  All services provided and paid for by PACE must be approved by the Interdisciplinary Team.
